Through Caregiver-Centered Care Education, continuing care staff and leaders explored how to better understand, include, and communicate with family caregivers. The learning led to real shifts—not just in knowledge, but in practice. Care teams became more confident in their conversations with families, and more intentional about partnering with them early and often.
A Culture of Partnership
As the work deepened, teams began rethinking daily routines, policies, and the ways family caregivers are welcomed into care settings. Leaders saw long-standing assumptions start to shift. Across many sites, caregiver partnership became not just encouraged but expected.
The Main Takeaway: A Comprehensive Toolkit for All
A key outcome of this effort is the "Embedding Caregiver-Centered Care" Toolkit. It was shaped directly by what providers, educators, and leaders said they needed in order to make lasting change. The toolkit brings together a wide range of practical materials: short videos, articles, posters, learning modules, conversation tools, and leadership supports. It gives organizations a clear starting point and a set of resources they can return to as their culture of caregiver partnership develops.
