The Alberta Caregiver-Centered Navigation Network (ACNN) is officially live. Launched by the Caregiver-Centered Care initiative, this vital provincial hub is designed to transform how navigation support to family caregivers is delivered. We bring together health, social, and community care sectors so that caregivers no longer have to navigate a complex system alone.

The ACNN is proudly supported by the Ministry of Seniors, Community and Social Services and anchored within the University of Alberta’s Department of Family Medicine.

Our Purpose

The network acts as a collective provincial voice to prioritize navigation needs, reduce cross- sector redundancy, and drive systemic change. ACNN is committed to ensuring navigation solutions are practical, accessible, and inclusive for all Alberta's family caregivers —including those in urban, rural, remote, and Indigenous communities.

Through our Best Practice Exchange, network members work together to:
Share What Works: Highlight successful local navigation strategies and scale them across the province.
Co-Develop Tools: Collaborate on practical, real-world pathways and resources to support navigation.
Build Successful Connections: Move beyond transactional referrals to establish navigation pathways that ensure caregivers are successfully connected.

Who Should Join?

We believe that meaningful system improvement requires balanced representation from across the province. We are seeking diverse perspectives from across four key areas:

Voices of Experience: Family caregivers and care recipients.
Voices of Practice: Frontline providers who offer navigation support.
Voices of Influence: System leaders and decision-makers.
Voices of Knowledge & Design: Researchers, educators, and facilitators.

Whether you are deeply involved in healthcare and community navigation or simply passionate about improving the lives of caregivers, your voice is welcome.

Join Us for the Virtual Kickoff Meeting
Our virtual kickoff meeting will focus on introducing the vision of the ACNN, connecting partners across sectors, and identifying our first collaborative opportunities to support family caregivers for collective action.

How to Participate:
Becoming a member of the network is entirely voluntary. Completing the Membership Sign-Up Form below is the first step. By signing up, you will secure your invitation to the Kickoff Meeting and receive updates on future bi-monthly gatherings, working sessions, and network communications.
